1 INDICATIONS AND USAGE Memantine hydrochloride tablets, USP are indicated for the treatment of moderate to severe dementia of the Alzheimer’s type. Memantine hydrochloride tablets, USP are an N-methyl-D-aspartate (NMDA) receptor antagonist indicated for the treatment of moderate to severe dementia of the Alzheimer’s type. ( 1 )

16 HOW SUPPLIED/STORAGE AND HANDLING Memantine Hydrochloride Tablets, USP are available containing 5 mg and 10 mg of memantine hydrochloride, USP. The 5 mg tablets are white, film-coated, round, unscored tablets debossed with M on one side of the tablet and 103 on the other side: NDC 0378-1103-91 bottles of 60 tablets NDC 0378-1103-05 bottles of 500 tablets The 10 mg tablets are white, film-coated, round, unscored tablets debossed with M on one side of the tablet and 104 on the other side: NDC 0378-1104-91 bottles of 60 tablets NDC 0378-1104-05 bottles of 500 tablets Store at 20° to 25°C (68° to 77°F). [See USP Controlled Room Temperature.] Dispense in a tight, light-resistant container as defined in the USP using a child-resistant closure. PHARMACIST: Dispense the Patient Information Leaflet with each prescription.
