METFORMIN HYDROCHLORIDE EXTENDED-RELEASE TABLETS are supplied as: 500 mg Bottles of 30 NDC 50228-445-30 white to off white, oval shape, film-coated tablet debossed with "445" on one side and plain on the other side. Bottles of 500 NDC 50228-445-05 Bottles of 100 NDC 50228-445-01 1,000 mg Bottles of 30 NDC 50228-446-30 white to off white, oval shape, film-coated tablets debossed with "446" on one side and plain on the other side. Bottles of 500 NDC 50228-446-05 Bottles of 90 NDC 50228-446-90 Store at 20°C to 25°C (68°F to 77°F); excursions permitted to 15°C to 30°C (59°F to 86°F) [see USP Controlled Room Temperature].
