INDICATIONS AND USAGE Hypertension Diltiazem hydrochloride extended-release capsules, USP are indicated for the treatment of hypertension. It may be used alone or in combination with other antihypertensive medications. Chronic Stable Angina Diltiazem hydrochloride extended-release capsules, USP are indicated for the treatment of chronic stable angina.

HOW SUPPLIED Diltiazem hydrochloride extended-release capsules, USP are available as follows: 120 mg – gray opaque cap and white opaque body printed with and “1112”on both the cap and body in black ink supplied in bottles of 30 (NDC 0781-2452-31) and bottles of 90 (NDC 0781-2452-92) with a child-resistant closure, and bottles of 500 (NDC 0781-2452-05) without a child-resistant closure. 180 mg – white opaque cap and body printed with and “1113”on both the cap and body in black ink supplied in bottles of 30 (NDC 0781-2453-31) and bottles of 90 (NDC 0781-2453-92) with a child-resistant closure, and bottles of 500 (NDC 0781-2453-05) without a child-resistant closure. 240 mg – white opaque cap and blue opaque body printed with and “1114”on both the cap and body in black ink supplied in bottles of 30 (NDC 0781-2454-31) and bottles of 90 (NDC 0781-2454-92) with a child-resistant closure, and bottles of 500 (NDC 0781-2454-05) without a child-resistant closure. 300 mg – blue opaque cap and body printed with and “1115”on both the cap and body in black ink supplied in bottles of 30 (NDC 0781-2455-31) and bottles of 90 (NDC 0781-2455-92) with a child-resistant closure, and bottles of 500 (NDC 0781-2455-05) without a child-resistant closure. 360 mg – blue opaque cap and gray opaque body printed with and “1116”on both the cap and body in black ink supplied in bottles of 30 (NDC 0781-2456-31) and bottles of 90 (NDC 0781-2456-92) with a child-resistant closure, and bottles of 500 (NDC 0781-2456-05) without a child-resistant closure. 420 mg – gray opaque cap and body printed with and “1117”on both the cap and body in black ink supplied in bottles of 30 (NDC 0781-2457-31) and bottles of 90 (NDC 0781-2457-92) with a child-resistant closure, and bottles of 500 (NDC 0781-2457-05) without a child-resistant closure. Store at 25ºC (77ºF); excursions permitted to 15 to 30ºC (59 to 86ºF) [see USP Controlled Room Temperature]. Avoid excessive humidity.
