INDICATIONS AND USAGE Perphenazine tablets are indicated for use in the treatment of schizophrenia and for the control of severe nausea and vomiting in adults. Perphenazine tablets have not been shown effective for the management of behavioral complications in patients with mental retardation.

HOW SUPPLIED Perphenazine Tablets, USP are available containing 2 mg, 4 mg, 8 mg, or 16 mg of perphenazine, USP. The 2 mg tablets are white to off-white, film-coated, round unscored tablets debossed with M on one side of the tablet and PZ over 2 on the other side. They are available as follows: NDC 0378-5350-01 bottles of 100 tablets The 4 mg tablets are white to off-white, film-coated, round unscored tablets debossed with M on one side of the tablet and PZ4 on the other side. They are available as follows: NDC 0378-5351-01 bottles of 100 tablets The 8 mg tablets are white to off-white, film-coated, round unscored tablets debossed with M on one side of the tablet and PZ8 on the other side. They are available as follows: NDC 0378-5352-01 bottles of 100 tablets The 16 mg tablets are white to off-white, film-coated, round unscored tablets debossed with M on one side of the tablet and PZ over 16 on the other side. They are available as follows: NDC 0378-5353-01 bottles of 100 tablets Store at 20º to 25ºC (68º to 77ºF).
